Meet The Team

Jarrod, Relationship Manager

Sarah De Lemos, Relationship Manager
I’m passionate about building genuine relationships and ensuring participants feel supported every step of the way. My goal is to make the NDIS feel more accessible and less overwhelming, while connecting people with services that align with their goals and values.
I have previously worked as an Access Assessor with the NDIA. This role gave me a deep understanding of the NDIS and the challenges individuals and families face when navigating the system for the first time.
Most recently, I worked at the Institute for Urban Indigenous Health (IUIH), where I supported Aboriginal and Torres Strait Islander people to access NDIS funding. I’ve been proud to contribute to creating culturally safe and appropriate pathways through a unique partnership between IUIH and The Benevolent Society—supporting families and children in a way that truly respects community needs.

Working as a Plan Manager at Manage Mode has given me the opportunity to create positive impacts for our participants and the talented professionals we work alongside with.
I previously worked as a Corporate Receptionist, Senior Administrator and Business Services Co-Ordinator. My sense of commitment to assist within the disability sector has led me to a fulfilling and gratifying career that aligns with my passion and values.
Having recently completed a Certificate IV Mental Health, I continue utilising this along with my developed skills to provide an inclusive level of service with compassion and integrity.

The NDIS can sometimes feel like a mine field for participants, their supports and providers. As my role, I hope to alleviate some of that stress and assist as best as I can , with the knowledge and experience I have gained across the various sectors.
